Who calls our houses home?
Six groups of neighbors, one shared table. Most residents belong to more than one of these.
Veterans
You held the line for others. A Heart to Heart house offers a stable base, a respectful community, and support services that come to you. Vouchers and VA-connected income sources are welcome in the application.
Seniors
Keep your independence without keeping a whole house running. Housekeeping, medication reminders, delivered prescriptions, and housemates who notice when you are not at breakfast.
Adults living with disabilities
Accessible rooms, visiting home health, non-emergency transportation, and light case management, arranged around how you actually live.
People transitioning between housing
Between leases, leaving an institution, or coming out of homelessness. A furnished room with utilities on is a real next step, not a waiting room.
Low-income individuals
Affordable rent with SSI, SSDI, vouchers, and private pay accepted. One monthly amount, no utility bills, no furniture to buy.
Independent and semi-independent adults
Adults who can manage daily life but want community and a safety net closer than a phone call across town.

Do you refer clients to housing?
Case managers, social workers, discharge planners, and veteran service officers use the same application form as families. Submit a referral online in minutes, and note yourself as the referring contact.
